What happened?

A worker was injured when a ball valve on a water line broke and he fell backward to get out of the way of the rush of water, injuring his back.

Why did it happen?

It was discovered that the valve was defective; a counterfeit part with fraudulent Canadian Standard Association (CSA) markings.

How could the incident have been prevented?

  • Educate workforce, including purchasing department, regarding counterfeit parts.

  • Inspect entire site or project for substandard valves.

  • Replace all substandard valves.
