Hazard Alerts are typically initiated and distributed under the following conditions:
1) An event (with injury or a near-miss incident) occurs
2) There is a learning opportunity for other companies with similar equipment or processes
Please complete the form below to initiate the hazard alert process. Alternatively, internal incident investigation forms and photos can also be submitted by e-mail. Please e-mail forms and photos to website@workplacesafetynorth.ca.
Next Steps
WSN will review the submitted information and contact the designated individual to confirm and discuss the incident. At this stage additional photos and documentation can be submitted to WSN.
WSN will write a draft Hazard Alert and forward it to the assigned contact name.
If the designate approves the hazard alert, WSN will post the alert to its website and distribute the hazard alert via its email distribution list.
